Description
This training provides reviews key intake forms to be completed within Sage as well as specialty forms form for specific population. This training is part of the Sage Onboarding requirements in order to obtain access to Sage-PCNX.

Learning Objectives
This training reviews the following forms and whether they are required and by whom: Admission (Outpatient), Update Client Data, Patient Handbook and Orientation Video Acknowledgment, Provider Site Admission, Women’s Health History, Reproductive Health, Client Picture, Collateral Contact, Patient Medication, Drug testing. 

Intended Audience
This training is intended for Primary and Secondary Sage users who enter intake information within Sage-PCNX.

This online course uses a mixed-media format with PowerPoint slides, video demonstrations, and closed captions.

Process
