Description
This training will present a general overview of various forms used within Sage-PCNX starting from when a patient first reaches out for Substance Use Disorder, or SUD treatment, to their admission into your agency, their time spent at your agency, and then their eventual discharge/transfer from your agency, location, or level of care.
Learning Objectives
During this training you will learn:
- 4 phases of treatment
- 2 additional processes that necessitate the use of Sage forms that happen during treatment
- Forms used in Sage starting from when a patient first reaches out for treatment through discharge/transfer
- When specific forms are used in treatment
Intended Audience
This training is appropriate for the SAPC provider Primary Sage User and Secondary Sage User network.
